Levels of Care - 'The Terrace'

Level I
• Resident performs most Activities of Daily Living (ADLs) without assistance.
• Resident is ambulatory with some assistance with cane or walker.
• All medications are managed by nursing staff.
• Health is generally good with no serious conditions.
• Resident is continent and does not require assistance.

Level II (Includes Level I services)
• Resident requires moderate reminders to verbal cues for ADLs and hands-on assistance with bathing.
• Mobility is unsteady with increased risk for falls. Resident uses cane or walker.
• Reminders for meals, improved intake and or intermittent nourishments are needed.
• Medications are managed by nursing staff and include blood glucose and insulin monitoring and minimal medication adjustments or changes.
• Health includes chronic stable non-life-threatening conditions.
• Resident needs occasional assistance with continence.
• Behavior may include judgment impairment. Resident may require reminding and redirecting.

Care levels exceeding Level II (will require an additional monthly charge)
Level III and higher care levels include one or more of the following:
Increased consistent assistance with ADLs, bathing and meals. Resident's mobility requires one person transfer from bed or chair to walker or wheelchair. Twenty-four hour assistance with incontinence. Behavioral problems that require consistent supervision and management. Physical health is declining and chronic conditions require close monitoring, increased management and possible Hospice intervention.*

*Hospice may require additional care giving support at an additional charge.
