Levels of Care - 'The Courtyard'
Level I
• Resident requires support with Activities of Daily Living (ADLs) to include hands-on bathing, hygiene, grooming, and dressing.
• Mobility requires occasional to consistent support.
• Meal monitoring requires prompting and occasional assistance.
• All medications are managed by nursing staff.
• Health includes chronic conditions that require minimal support and management.
• Resident is continent the majority of the time but needs reminders, cues and some assistance.
• Behaviors include short term memory loss, judgment impairment, and wandering. Resident requires reminders and redirection.
Level II (Includes Level I services)
• Resident requires additional assistance with ADLs, bathing, hygiene, grooming and dressing.
• Mobility requires one person transfer from bed to standing to using a walker, or wheelchair assistance.
• Resident requires more consistent assistance with eating meals.
• Medication management includes more frequent medication adjustments and changes.
• Medications include blood glucose and insulin monitoring.
• Chronic health conditions require increased support and management.
• Incontinence requires 24 hour care.
• Behavior includes significant judgment impairment with increased supervision for wandering and redirection. Some interventions for difficult behaviors are required.
Care levels exceeding Level II (will require an additional monthly charge)
When an individual needs a higher level of care than outlined above, monthly charges will increase. A higher level of care will be indicated by one or more of the following:
Total assistance with ADLs, bathing, meals, transfers and mobility, crushed meds, total incontinence, constant behavioral supervision, significant physical decline, possible Hospice intervention.*
*Hospice may require additional caregiving support and additional charges.
